But if you would like a copy of your dental records emailed to you please fill in your details below. One of our team members will contact you as soon as possible. You must fill in all required fields.

Subject Access Request
Under the Data Protection Act 2018 and the General Data Protection Regulations (GDPR), you have the right to make a request for a copy of the personal information that Dental Care Ireland processes about you. This is called a Subject Access Request (SAR). We will provide a response within one month. We may contact you, using the details below, should we have any questions about your request.
The details you provide will be used to verify your identity and must match the details registered on your Dental Care Ireland chart. Incorrect information may delay our ability to fulfill your request.
